A valid prescription for contact lenses should contain name of patient, date of examination, full name of optometrist and phone number, signature of optometrist, parameters of contact lenses and prescription expiry date (not more than 12 months from the date of examination).

If your uploaded prescription details are incomplete, you are required to have an eye examination in one of our stores. No shipping and refund will be done for order with incomplete prescription details.

Each account is applicable only for personal prescription and purchase records. Purchasing contact lenses with another customer's prescription is not allowed.
